This page is the authoritative index of the documents governing use of the ApalyRx platform. Platform use is governed by the ApalyRx Terms of Use together with the Addendum for your role. Accepting the Terms and your Addendum is what governs your use of the platform.

Master Agreements

The Terms of Use and the Privacy Policy apply to every user of the platform, in every role. The Business Associate Agreement applies only to organizations that are covered entities under HIPAA, or that are business associates delegating a function to Apaly Health Inc. It does not apply to individual members.

The documents below are separate instruments. Each is presented separately, requires its own affirmative acceptance, and is not incorporated into the Terms of Use or any Addendum. Accepting the Terms of Use does not give any authorization or consent, and declining any of these does not affect your acceptance of the Terms.

Member Instruments

HIPAA Authorization

Authorization to use and disclose protected health information for program participation under 45 C.F.R. Section 164.508. Separate document, separately accepted, revocable at any time.

TCPA Communications Consent

Express written consent to receive automated calls, prerecorded voice messages, and text messages. Separate document, separately accepted, withdrawable at any time.

Consent to prescription routing and pharmacy selection, government program acknowledgments, and program exclusivity rules. Separate document, separately accepted.

Supplier and Third Party Forms

A supplier, dispensing pharmacy, or other third party may impose its own terms on your access to its products, services, or programs.

Those terms are between you and that party and are available from that party. Each requires its own affirmative acceptance. Apaly Health does not adopt, host, or become responsible for them, and no such form is incorporated into the Terms of Use. See Section C6 of Addendum C and Section 10 of the Terms.

Commercial participation in the ApalyRx program is governed by a separately signed agreement between the organization and Apaly Health Inc. Those agreements are executed off platform and are not published here. Questions: legal@apalyrx.com.
